Apply for Medicare at SSA.gov

If you do not already have a Social Security account, you can create an account and apply for your Part A and/ or B online. You do not have to take SS when you apply for Medicare. 1-800-772-1213 if you would rather apply in person.

Medicare Account at Medicare.gov

Creating a Medicare.gov account once you have Medicare allows beneficiaries to manage coverage, view claims, pay premiums, and print their Medicare card.  They can also track expenses,update personal information and compare Medicare coverage options.  1-877-436-0234

Apply for Part B Only

For those who didn’t apply for their Part B during their Initial Enrollment Period, you can use this form to apply for Part B at a later time.

Proof of Prior Coverage

If you are applying for Medicare after your Initial Enrollment Period, you will need this form completed by the Human Resources Dept. associated with your current employee coverage. This form will need to be submitted at the time of your application for part B.

Life Changing Event

This form is used to appeal the IRMAA (Income Related Monthly Adjustment Amount) for Part B, if there has been a significant change in your life, such as retirement , loss of income, etc.

Working Past 65

This is a helpful site that can answer your questions about the need to take your Part B if you or your spouse are still working and have coverage. This is important in avoiding lifelong penalties.
